Revolutionizing Postoperative Recovery: The ERABAS Protocol by Dr. Juan Manuel Seren In a context where Argentina records significant activity in plastic surgery — with more than 59,000 breast procedures performed annually according to the latest report from the International Society of Aesthetic Plastic Surgery (ISAPS) — a doctor from Buenos Aires is revolutionizing the way postoperative recovery is understood. Juan Manuel Seren (MN 107,174), a plastic surgeon specialized in breast surgery, has developed a protocol that challenges everything that was believed possible: that patients can resume their normal lives just 24 hours after a breast augmentation surgery, without significant pain, drains, or compressive dressings. His protocol ERABAS (Enhanced Recovery After Breast Augmentation Surgery) is not only changing the experience of thousands of Argentine women, but it is also attracting the attention of specialists worldwide after being published in the prestigious international journal Aesthetic Plastic Surgery. "My main challenge was to demonstrate the truth, that is, scientifically validate the method. Today, rapid recovery is science and not fiction," explains Seren. The science backs it up: a retrospective study of over 10 years involving 2,906 patients showed that 95.32% achieved complete functional recovery within the first 24 hours. To validate these results, Seren and his team conducted a thorough bio-statistical analysis with two binary logistic regression models, evaluating both recovery time and postoperative pain. The results were published in the prestigious international journal Aesthetic Plastic Surgery. "With this protocol, there are no drains, no compressive garments, there is immediate mobility after surgery. Even, one can drive a vehicle between the first 24 to 48 hours," details the specialist. Patients can shower, wear normal bras, and maintain full arm mobility from the first day. A technique that transcends borders. The ERABAS protocol incorporates ten fundamental principles ranging from total intravenous anesthesia with short-acting drugs to the complete elimination of opioids in the postoperative period. But perhaps one of the most innovative aspects is the new dissection sequence that Seren described worldwide. "We have developed a sequence that allows breast augmentation surgeries without intraoperative bleeding. By creating a bloodless pocket, we significantly reduce surgical time, allowing a breast augmentation to be completed in less than 40 minutes," explains the surgeon. This technique, which specifically identifies and controls three arterial vessels (AICAP, MICAP, and LICAP), allows bloodless breast surgeries, dramatically accelerating recovery. Graduating with honors from the University of Medical Sciences of Buenos Aires and specializing in plastic surgery at the Pontifical Catholic University of Argentina with an average of 10, Seren leaves nothing to chance. His statistical analysis identified the exact profile of patients who can benefit the most from the protocol. "Those patients undergoing breast augmentation with a subfascial pocket, who are under 35 years old, receive breast implants less than 350 cubic cm, and do not smoke, have a 98.9% recovery in the first 12 hours," he explains. This means that a young and healthy woman with these characteristics can resume her work and household activities the same afternoon of the intervention, with a single restriction: avoid physical activities that generate tachycardia. Demystifying Breast Surgery In a country where breast surgery has a significant presence in the landscape of aesthetic procedures — Argentina records significant volumes, according to ISAPS — Seren considers it important to educate patients about the myths surrounding these interventions. "A very common myth is that ‘I undergo surgery and breast surgery is for life.’ It is not. Breast prosthesis surgery represents a stage in a woman’s life, not a definitive result," he clarifies. Hormonal changes, weight changes, pregnancies, and the passage of time influence the appearance of the breasts, so he considers it more accurate to think of surgery "as an instant photograph, rather than a complete film." Another persistent myth is the obligation to change implants every 10 years. "This notion is incorrect. The replacement of prostheses is determined by the individual needs of the patient and the condition of the implant," he emphasizes. Scientific advances have allowed the creation of much lighter and more functional implants. An example of these advances is the use of a gel that weighs up to 30% less than previous models. This innovation reduces the effect of gravity from the implant on the breast tissues. "Surgical breast procedures have evolved and vary according to the needs of each person. Among the most common are breast augmentation, breast reduction, breast reconstruction for oncological patients who have undergone a mastectomy, and mastopexy, which lifts sagging breasts. In addition, transgender surgery allows patients to make the physical transition to their self-affirmed gender," details Seren. A Global Reference from Buenos Aires With over 20 years of experience, Seren was inspired by the concept of Fast Track Surgery developed by Danish doctor Henrik Kehlet in 1997, but he adapted it specifically for breast surgery. His commitment to research and scientific validation led him to share his findings with the international medical community. "Any certified and trained plastic surgeon can achieve a fast, simple, and reproducible recovery, prioritizing patient safety and well-being," he assures, emphasizing that his goal goes beyond individual success to become a global standard of care. In a context where Argentine plastic surgery has internationally recognized professionals — trained in excellent institutions with cutting-edge techniques — Seren’s ERABAS protocol stands as a new milestone that once again places Argentine medicine on the world map of innovation.
